Home Closing
Is Your Home Closing? – if the Social Work Department and/or DSS contribute to the funding of your care, you will be visited by a social worker who will carry out a needs/financial assessment.
You (and your relatives/carers) may be given advice on where there are available places in other homes and you will be asked to choose. Visits to other homes can be made with your social worker or a relative or an advocate may make a visit on behalf of someone unable to make a choice for themselves.
If you are a private/self-funding resident, you and your relatives may be given information on other homes so that you may choose another placement with the help of your relatives.
Why would you need an advocate?
- You may have no one else to help you with making decision about your move.
- You may feel that you are not able to speak up for yourself or are not being given the opportunity to do so.
- You may feel pushed into making decisions by others.
- You may feel that no one is listening to what your are saying about what it is that you want.
- You may be unhappy about your move or about the decision after it has happened.
