Delayed Discharge
Delayed Discharge (sometimes referred to as ‘Awaiting Discharge’) – the service is for older people who have been assessed as being ready to move on from hospital and may need someone independent to speak or act on their behalf or are at risk or have experienced, for example, isolation, exclusion or mistreatment.
Why might you need this service?
- Your are ready to move from hospital and are seeking other accommodation
- The NHS needs to free your bed and you are facing decisions about where you want to move to
- The ward may be closing
- You have a place in a care home or sheltered housing and are waiting to move
- You may be finding it difficult to express your wishes/views to those involved in your care
- You may disagree with decisions that have been made by the people looking after you
